Cremation Services
One may book in person or authorise a licensed undertaker of burials to book on his/her behalf a cremation session at the 6 crematoria under the FEHD.
When submitting an application for cremation services, an applicant who chooses to book a cremation session online through the FEHD website will be given a token upon completion of payment at the FEHD counter of a Joint Office. The applicant may then use the token to book a cremation session online within 60 days.
Application procedures for cremation services

1. Go through the formalities at the FEHD counter of a Joint Office and submit/produce the following documents:
- a completed "Application for Private Cremation" (FEHB 135) (the particulars of the appointed licensed undertaker must be provided in the form, which must also be signed by the undertaker and stamped with the undertaker's chop recognised by the FEHD)
- the "Cremation Permit" (Form 3) or "Certificate of Order Authorizing Cremation of Body" (Form 11) (commonly known as "Cremation Order") (please refer to "Step 1 - A Guide for the Bereaved")
- the original identity document of the deceased
- the original identity document of the applicant
2. The documents being checked by the FEHD staff
3. Pay the cremation fee
4 (i). Book a cremation session immediately at the counter; or
or
4 (ii). Be given a token for online booking of cremation session
(For applicants who opt to book a cremation session online)
5. Be issued with a receipt of cremation fee payment
6. Bring along the original of the following documents to the crematorium concerned according to the date, time and venue booked:
- the "Cremation Permit" (Form 3) or "Certificate of Order Authorizing Cremation of Body" (Form 11); and
- the receipt of cremation fee payment or the acknowledgement receipt of the booking of cremation services
7. Bring along the receipt of cremation fee payment or the acknowledgement receipt of the booking of cremation services to the selected venue for the collection of cremated ashes after the cremation
(Storage of ashes is free of charge for 60 days from the date of cremation, and a fee will be charged for the uncollected ashes after the free storage period.)
